Volunteer Enlistment
State of New York Town of Westmoreland I, George B. Fredericks, born in New York in the state of New York, aged 39 years, and by occupation a Cordwainer, Do Hereby Acknowledge to have volunteered this 26 day of August, 1864 to serve as a Soldier in the Army of the United States of America, for the period of one year, unless sooner discharged by proper authority: Do also
agree to accept such bounty, pay, rations, and clothing, as are, or may be, established by law for volunteers. And I, George B.
Fredericks solemnly swear, that I will bear true faith and allegiance to the United States of America, and that I will serve them honestly and faithfully against all their enemies or opposers whomsoever: and that I will observe of the officers appointed over me, according to the Rules and Articles of War.

I certify, on honor, that George B. Fredericks, a Corporal of
Captain C. K. Baker's Company (H) of the 100th Regiment of New York
State Inf. Volunteers, of the state of New York, born in New York,
state of New York, aged 39 years; 5 feet 5 inches high; light complexion, blue eyes, dark hair, and by occupation a Cordwainer, who joined for service and was enrolled (see Note 9) on the 26th day of August, 1864, at Westmoreland, NY by R. H. Richardson, for the period of one year, and mustered into the service of the United States on the 26th day of August, 1864, at Utica, NY by Capt. Richardson; and having served honestly and faithfully with his Company to the present date, is now entitled to a Discharge by reason of Death - Killed in assault on Ft Gregg, near Petersburg, VA April 2, 1865
The said George B. Fredericks was last paid by Paymaster Maj. Dorman, to include the 31st day of December, 1864, and has pay due him from that time to the present date; he is entitled to pay and subsistence for traveling to place of enrollment, and whatever other allowances are authorized to volunteer soldiers, drafted men, or milita, so discharged. He has received from the United States clothing amounting to 25. 30 dollars, since the 26th day of August, 1864, when his clothing account was last settled. He has received from the United States 33.33 dollars advanced Bounty.
